Indications

Symptomatic treatment of mild, moderate and severe Alzheimer’s type dementia.

Contraindications

Hypersensitivity to donepezil, other components of the drug, as well as to piperidine derivatives; – the efficacy and safety of donepezil in patients under the age of 18 has not been established; – lactase deficiency, lactose intolerance, glucose-galactose malabsorption.

With caution

Cholinesterase inhibitors should be prescribed with caution to patients with a history of obstructive pulmonary disease (including with bronchial asthma), with cardiac arrhythmias (possibly vagotonic effect on heart rate, in particular bradycardia), with anesthesia, as well as patients with an increased risk of peptic ulcer development (for example, patients with a history of peptic ulcer disease or receiving concomitant therapy with nonsteroidal anti-inflammatory drugs), although no increase in the frequency of peptic ulcers or gastrointestinal bleeding was detected when using Arisept® (see “Special Instructions). Simultaneous use of donepezil with other acetylcholinesterase inhibitors, agonists or antagonists of the cholinergic system should be avoided.

Dosage regimen

Adults (including elderly patients) the drug is prescribed at an initial dose of 5 mg 1 time / day. The initial dose is continued for at least 4-6 weeks to achieve equilibrium concentrations of donepezil and determine the early clinical effect of therapy. After 1 month, the dose of Arisept can be increased to 10 mg / day, which is the maximum recommended daily dose. Maintenance therapy can be continued as long as the therapeutic effect persists, which should be evaluated regularly. In case of impaired liver and kidney function, dose adjustment is not required.
