Pharmacological action

Sulfasalazine (salazosulfapiridine) has immunosuppressive, anti-inflammatory and bactericidal effects. Its action is the result of the influence of two metabolites that locally affect the intestinal wall, at the same time, having a systemic effect. Sulfapyridine inhibits the action of natural killer cells (NK cells) and the transformation of lymphocytes. The anti-inflammatory effect of 5-aminosalicylic acid is perhaps the most important property in the treatment of inflammatory bowel diseases, since it inhibits cyclooxygenase and lipoxygenase in the intestinal wall and thereby prevents the formation of prostaglandins, leukotrienes and other inflammatory mediators, and also probably binds oxygen free radicals. Sulfasalazine does not have an analgesic effect.

Overdose

There is evidence that the frequency and severity of toxicity after overdose is directly related to the total serum concentration of sulfapyridine. Overdose symptoms may include nausea, vomiting, gastric distress, and abdominal pain. In more complex cases, symptoms of the central nervous system, such as drowsiness, seizures, etc., may be observable. Serum sulfapyridine concentrations can be used to monitor the progress of recovery from overdose. There are no documented reports of death due to taking large single doses of sulfasalazine. Doses of azulfidine tablets of 16 g per day were given to patients without mortality. A single oral dose of 12 g/kg was not fatal for mice. Overdose Instructions Gastric lavage or vomiting plus catharsis, as indicated. Alkaline urine. If kidney function is normal, force fluids. If anuria is present, limit liquids and salt and treat accordingly. Catheterization ureters may be indicated for complete renal blockage by crystals. The low molecular weight of sulfasalazine and its metabolites may facilitate their removal by dialysis.
